DulaglutideGLP-1 receptor agonist medicine
Dulaglutide is a GLP-1 receptor agonist used in type 2 diabetes. Its evidence should be read through glucose control, relevant clinical outcomes and tolerability rather than a generic appetite-suppression claim.
What is Dulaglutide?
In the EU, Trulicity is indicated from age 10 for insufficiently controlled type 2 diabetes alongside diet and exercise: alone when metformin is inappropriate because of intolerance or contraindications, or added to other diabetes medicines. 1
How it works
It activates GLP-1 receptors, supporting glucose-dependent insulin secretion and other processes involved in glucose regulation. The response still depends on the rest of the treatment regimen. 1
Effects studied
Glucose control in type 2 diabetes
Clinical trials assess glucose control, including HbA1c, in particular treatment combinations. HbA1c does not by itself describe weight, cardiovascular or kidney outcomes.
Evidence: Diabetes trial programme and EMA product information. 1
REWIND
REWIND enrolled 9,901 adults aged 50 or older with type 2 diabetes and cardiovascular disease or risk factors. Over a median 5.4 years, cardiovascular death (including unknown causes), non-fatal myocardial infarction or non-fatal stroke occurred in 12.0% with weekly dulaglutide 1.5 mg versus 13.4% with placebo: HR 0.88 (95% CI 0.79–0.99). 4
Evidence: This composite outcome does not establish a benefit for every component or for people without diabetes. All-cause mortality was not significantly different. Kidney findings come from separate analyses, described in the kidney entry. 4
What the separate kidney analysis suggests
Kidney outcomes deserve their own discussion. A 2023 post hoc analysis of REWIND found fewer events in a composite of sustained loss of kidney function, end-stage renal disease or kidney-related death with dulaglutide. This supports a possible kidney benefit in the studied diabetes population, but the analysis was post hoc and does not establish a benefit for each component or for people without diabetes. 6
Evidence: Post hoc analysis of a randomised trial; separate from the primary cardiovascular outcome.
What does the evidence show?
Type 2 diabetes trials and EMA product information
Working with a meal-related pathway
Dulaglutide activates the GLP-1 receptor. This supports glucose-dependent insulin secretion and affects other processes involved in glucose regulation. The glucose-dependent aspect helps explain its pharmacology, but does not remove the need to consider the person’s other diabetes medicines. 1
The clinical question is specific
In the EU, Trulicity is indicated from age 10 for insufficiently controlled type 2 diabetes alongside diet and exercise: alone when metformin is inappropriate because of intolerance or contraindications, or added to other diabetes medicines. 1
Different outcomes provide different information

This composite outcome does not establish a benefit for every component or for people without diabetes. All-cause mortality was not significantly different. Kidney findings come from separate analyses, discussed below. 4

Tolerability and combination treatment
Nausea, vomiting and diarrhoea may cause dehydration and worsen kidney function. Insulin or sulfonylureas increase hypoglycaemia risk. Persistent severe abdominal pain requires urgent assessment for pancreatitis. 5
Tell the anaesthesia team about treatment before general anaesthesia or deep sedation because gastric emptying is delayed. Severe gastroparesis requires particular consideration; this overview does not replace the full prescribing information. 5
The US label carries a boxed warning based on thyroid C-cell tumours in rats; relevance to humans is unknown. It contraindicates use with a personal or family history of medullary thyroid carcinoma or MEN2. These are US-label statements. 2

Frequently asked questions
Does a diabetes indication also mean a weight-management indication?
No. Those are distinct treatment purposes and require their own product indications. 1
What does HbA1c tell us?
It reflects average glucose exposure over time. It does not replace assessment of other outcomes or the daily treatment experience.
