Liver

A guide to liver tests, fatty liver and what supplement trials actually measured.

What does a healthier liver mean?

This page looks at fatty liver, liver blood tests and supplement research. It does not assess every cause of liver disease.

Fat, inflammation and scarring are different findings

A fatty liver has an increased amount of fat stored in its tissue. Diabetes, excess weight, alcohol and certain medicines can contribute; early fatty liver may improve when its cause is addressed. Untreated disease can progress to inflammation and tissue damage. 1

The newer name MASLD describes fatty liver associated with metabolic risk factors, such as type 2 diabetes. Older studies usually call it NAFLD. When inflammation and liver-cell injury accompany the fat, the newer term is MASH, previously NASH; fibrosis means scarring. 2

What do liver tests actually measure?

Blood tests, scans and sometimes a biopsy answer different questions. ALT and AST are enzymes measured in blood; raised levels can prompt investigation. A routine ultrasound can identify fat, while elastography measures tissue stiffness to help assess scarring. 3

A lower ALT result deserves context. IQWiG notes that liver disease, medicines and even strenuous exercise can raise it. The clinical question is why it changed, rather than whether a supplement can move the number into the reference range. 4

Why the cause comes first

Alcohol-related disease, viral hepatitis, inherited iron overload and immune-related disease need different assessments and treatments. Many liver conditions cause no symptoms early on, so feeling well does not settle the question. 5

For suspected fatty liver, clinicians also consider diabetes, blood fats, activity, diet and alcohol intake. Blood-based scores such as FIB-4 and stiffness measurements help assess the risk of advanced scarring. 3

When does this need medical attention?

Unexplained abnormal liver tests warrant assessment of their cause, including a review of medicines and herbal products. A supplement trial should not delay that assessment. 4 6

In someone with cirrhosis, yellow skin or eyes, swelling or breathlessness require urgent assessment. Vomiting blood, black stools or sudden confusion require emergency care. 7

Can supplements improve liver disease?

The 2024 European MASLD guideline does not recommend nutritional supplements as treatment: evidence for meaningful liver outcomes and safety is inadequate. That conclusion leaves room for investigating promising findings, but it does not establish a routine supplement regimen. 2

Milk thistle: why a positive secondary result is not the whole answer

Silymarin is a mixture of compounds in milk thistle extract. Across liver conditions, NCCIH describes the trial results as conflicting or too limited for firm conclusions. 8

SilymarinMixed

In a placebo-controlled trial, 99 adults with biopsy-confirmed NASH received 700 mg three times daily or placebo for 48 weeks. The main disease-activity outcome did not improve significantly more with silymarin. A secondary scarring measure favored it, but the researchers called for confirmation in a larger trial. This is a research dose, not a self-treatment recommendation. 9

SAMe: laboratory changes without a clear long-term benefit

The relevant research includes people with several different liver diseases. It therefore cannot be read as one treatment result for everyone with an abnormal liver test. 10

S-adenosylmethionine (SAMe)Too little evidence

A 2015 meta-analysis included 12 randomized trials involving 705 patients. Some comparisons favored SAMe for bilirubin or AST, but ALT and long-term prognosis did not show a significant advantage. Different illnesses and treatment regimens limit how directly the pooled findings apply to a particular patient. 10

What has a firmer place in care?

For MASLD, European guidance puts diet quality and regular activity at the center of care. It favors a Mediterranean-style pattern and fewer sugary drinks and highly processed foods. Exercise can reduce liver fat even when weight loss is not the main result. 2

When excess weight is present, sustained weight reduction can improve fatty liver. This is different from advising everyone with liver disease to diet: advanced disease can involve malnutrition, so nutrition care needs to fit the diagnosis. 2 7

Treating the cause remains essential. Some viral hepatitis needs medicines, and alcohol-related liver disease may improve when alcohol use stops. These interventions address the illness itself rather than a blood-test number. 5

What should be checked before using a liver supplement?

Milk thistle may cause digestive symptoms or allergic reactions, particularly in people allergic to related plants. Products have also shown discrepancies between stated and measured silymarin content and contamination. Pregnancy and breastfeeding safety is poorly understood. 8

SAMe can worsen mania in bipolar disorder and may interact with antidepressants or levodopa. Long-term safety and pregnancy safety remain uncertain. With cirrhosis, the NHS advises checking medicines, including herbal medicines, with a pharmacist or doctor. 6 7

Which products are outside this review?

This page does not evaluate every cataloged ingredient. In particular, it does not assign a liver benefit to minerals, amino-acid blends or the other herbal products not discussed above.

Questions people ask

Can a normal-looking scan rule out liver scarring?

Routine imaging can show fat but cannot reliably determine inflammation or fibrosis. Stiffness measurements and blood-based fibrosis scores answer different questions; sometimes a biopsy is needed. 3

Does a lower ALT mean the liver is healed?

ALT is one part of the assessment. Its level can change for several reasons, and it does not replace an assessment of scarring or the underlying cause. 3 4

Can early fatty liver improve?

Yes. When the cause is addressed, early fatty liver can regress. The approach depends on contributors such as alcohol, diabetes, excess weight or medicines. 1

Is milk thistle a proven liver treatment?

Human trials are mixed. One trial found a possible scarring benefit but missed its main disease-activity outcome; NCCIH considers the overall evidence insufficient for firm conclusions. 8 9

Should herbal products be mentioned when liver tests are abnormal?

Yes. IQWiG advises discussing medicines, including herbal products, because they can contribute to raised liver enzymes. 4
