Male Libido
Less interest in sex can raise questions about relationships, health, and hormones. This guide looks at those questions and checks what maca and fenugreek studies actually measured.
Is low desire the same as an erection problem?
Libido means sexual interest. Trouble getting or keeping an erection is a separate concern, although it can lead someone to avoid sex and lose interest. A useful assessment considers both. 1
What can change your sex drive?
Stress, depression, relationship difficulties, alcohol, some medicines, and illnesses such as diabetes can contribute. Treatment depends on the cause; counseling or a medication review may be more relevant than a supplement. 2
Does low desire mean low testosterone?
Testosterone influences sexual desire, but a laboratory result needs interpretation alongside symptoms and other findings. IQWiG notes that reference ranges vary and a low result can be temporary. 3

The studies below measure different things: reported sexual desire and hormone concentrations. 5 6
A systematic review of four trials found limited evidence for sexual function. Results in healthy men included improvement in desire in one trial and no clear benefit in a small cycling study. Small samples and study quality prevented firm conclusions. 5
In a 2024 trial, 95 men aged 40–80 completed 12 weeks of placebo or 600–1,800 mg daily of a specific extract. Salivary testosterone increased compared with placebo, but participants reported no subjective improvement. A hormone change therefore did not establish a libido benefit. 6
Do these results apply to every product?
The fenugreek trial tested one formulation, with vitamins and minerals also supplied to the comparison group. Its finding cannot establish that another blend will improve sexual desire. 6
What can a medical conversation clarify?
A discussion can cover when desire changed, relationship circumstances, other sexual problems, and medicines. Testosterone testing can be useful in selected assessments; treatment of a contributing condition may also improve desire. 1
Questions people ask
When is it worth seeking help?
The NHS recommends an appointment if low sex drive worries you or you suspect a medicine is involved. 2
Does more testosterone in a study mean more desire?
Not necessarily. The 2024 fenugreek trial found a salivary testosterone change without subjective improvement. 6
Is maca proven to restore libido?
No. The systematic review found the trials too small and methodologically limited for a firm conclusion. 5
Can relationship concerns matter?
Yes. UCSF includes relationship circumstances and communication in the assessment of low desire. 1
Are natural sexual-enhancement products automatically safe?
No. NCCIH warns about hidden drug ingredients and potentially serious interactions. 4
