Menstruation Support

A guide to premenstrual symptoms, period pain, and the evidence behind cycle-support supplements.

What needs support: the days before, or the period itself?

PMS follows a pattern: symptoms appear before a period and ease as bleeding begins. Irritability, breast tenderness, and fatigue can belong to this pattern. Keeping a symptom diary for two to three months helps a clinician distinguish PMS from problems that persist throughout the month. 1

When cramps deserve a closer look

Period pain often starts just before or with bleeding. Increasing pain, heavier or irregular periods, or bleeding between periods warrants assessment. Endometriosis and fibroids are among the possible underlying causes; severe or unusually bad pain that has not responded to painkillers needs urgent medical advice. 2

Can chasteberry help with PMS?

A systematic review included eight PMS trials: seven favored Vitex extracts over their comparator, including five of six placebo comparisons. The direction is encouraging, but some trials were small and preparations and comparisons varied. Those results do not establish one reliable benefit size for every product. 3

Chasteberry (Vitex)Too little evidence

NCCIH describes the PMS evidence as low to moderate quality. Breast tenderness is one of the symptoms that might improve. 4

How do other supplements compare?

IQWiG finds weak indications of benefit for calcium, while results for magnesium and evening primrose oil are conflicting. This leaves room for careful discussion, but does not justify treating a cycle-support blend as a proven package. 5

Evening primrose oilMixed

Trials have produced inconsistent PMS results. 5

What else can make the month easier?

Regular exercise may reduce PMS symptoms, according to the studies summarized by IQWiG. For severe psychological symptoms, prescription options include certain antidepressants; treatment choice depends on the symptoms and individual circumstances. 5

Scope of this review

This page focuses on PMS and period pain. It does not review fertility, menopause, DIM, fennel, or every ingredient listed in the directory.

Herbal does not mean suitable for everyone

Chasteberry can cause nausea, abdominal pain, diarrhea, headache, or itching. NCCIH cautions about pregnancy, breastfeeding, and hormone-sensitive cancers. People taking medicines should discuss herbal products with their clinician because interactions are possible. 4

When symptoms interfere with life

The NHS advises seeking care when PMS affects daily life or lifestyle measures have not helped. Options include hormonal contraception, talking therapy, and antidepressants; a supplement is only one part of the conversation. 6

Questions people ask

Does PMS mean my hormone levels are abnormal?

Not necessarily. IQWiG describes sensitivity to normal hormonal changes as a possible explanation; the causes remain incompletely understood. 1

Does chasteberry also improve fertility?

There is not enough reliable evidence to establish a fertility benefit, and pregnancy is a safety concern. 4

Is PMS the same as cramps during a period?

They overlap in everyday language, but PMS follows the premenstrual pattern, while period cramps often start around the beginning of bleeding. 1 2

What can help ordinary period pain?

The NHS lists warmth, gentle exercise, and pain relievers among options. Persistent or worsening pain calls for assessment. 2

Can I judge a change after one cycle?

Symptoms can vary between cycles. A diary over several months gives a clearer record of timing and severity. 1
