Gut Integrity

Gut integrity describes the intestinal barrier. Increased permeability is real, but a general “leaky gut syndrome” is not an established diagnosis, and supplement results depend on the condition studied.

What does gut integrity mean?

Your intestinal lining lets water and nutrients pass while helping keep bacteria and other harmful material out. Increased intestinal permeability means that this barrier lets more through than usual; it occurs in conditions including celiac disease and inflammatory bowel disease. 1

Does bloating mean my gut is leaking?

Bloating alone cannot establish a barrier problem. Irritable bowel syndrome (IBS) can cause abdominal pain, diarrhea or constipation without visible damage to the digestive tract, so symptoms and tissue damage are different questions. 2

What do permeability tests measure?

Research often measures how much of a swallowed sugar solution later appears in urine. That is an indirect measure of passage across the intestinal lining. Uncertain normal ranges and inconsistent procedures limit interpretation; a better result does not automatically mean better health. 3

When does the cause need checking?

Blood in the stool, fever, marked weight loss or pallor point toward causes other than uncomplicated IBS. These symptoms deserve medical assessment rather than a supplement experiment. 4

Celiac disease usually requires blood tests and sometimes intestinal biopsies. Starting a gluten-free diet before testing can change the results, which is why doctors advise waiting until the diagnostic assessment. 5

Which supplements have human evidence?

This section looks at glutamine, zinc carnosine and probiotics. It does not review the other supplements listed in the directory.

L-glutamineMixed

A 2024 meta-analysis found no significant overall improvement in intestinal permeability. Trials differed in participants, duration and methods; this does not support a universal “gut repair” dose. 6

Zinc-L-carnosineToo little evidence

Ten healthy volunteers completed a crossover trial with indomethacin, an anti-inflammatory medicine. Zinc carnosine, 37.5 mg twice daily, prevented the rise in permeability seen with placebo over five days. This small drug-challenge study does not establish lasting symptom relief. 7

ProbioticsToo little evidence

NCCIH reports possible benefits for IBS symptoms, but uncertainty about which strains help. Symptom findings do not establish that an interchangeable probiotic product repairs the intestinal barrier. 8

What was different about the positive glutamine trial?

Adults with post-infectious, diarrhea-predominant IBS and increased permeability received 5 g three times daily or placebo for eight weeks. Among completers, 43 of 54 glutamine recipients versus 3 of 52 placebo recipients achieved the prespecified symptom improvement. Permeability also improved. Larger trials are needed before generalizing beyond this selected group. 9

What matters beyond supplements?

For IBS, care may combine dietary changes, medicines and psychological therapies. Different people respond to different approaches, so the aim is a workable plan for symptoms rather than a single “integrity” score. 2

When celiac disease or inflammatory bowel disease damages the barrier, treatment needs to address that disease. Normalizing permeability alone is not an established cure. 3

What should I know before trying a product?

Probiotics carry greater risks in people with severe illness or weakened immune systems, including infections. NCCIH advises weighing benefits and risks carefully in these groups. 8

The glutamine IBS trial reported no serious adverse events over eight weeks. That short observation cannot establish long-term safety for other populations or higher doses. 9

Questions people ask

Is leaky gut a real condition?

Increased intestinal permeability is real. The wider “leaky gut syndrome” promoted as an explanation for many illnesses is not an established diagnosis. 1

Can a urine sugar test diagnose the cause of my symptoms?

It measures an aspect of permeability. Limitations in standardization and normal ranges mean it cannot, by itself, explain digestive symptoms. 3

What dose of glutamine repairs the gut?

Mixed clinical findings have not established a universal dose for gut repair. 6

Should I avoid gluten before getting tested?

A gluten-free diet can interfere with celiac testing. NIDDK advises against starting it before diagnostic testing. 5

Are probiotics safe for everyone?

No. Severe illness or a weakened immune system increases the risk of harmful effects, including infection. 8
