Testosterone Support

What does a low testosterone result mean, and can a supplement help? This guide covers diagnosis, botanical research, and the difference between a hormone measurement and treatment of a deficiency.

What does testosterone do?

Testosterone contributes to sexual desire, sperm production, and muscle and bone development. Levels vary with age and time of day, and laboratory reference ranges differ. 1

When does a low result need treatment?

The Endocrine Society requires compatible symptoms and consistently low testosterone for a diagnosis of hypogonadism. It recommends confirming the result with another morning, fasting measurement and investigating the cause. 2

Could the result be temporary?

Yes. IQWiG notes that stress or prolonged physical exertion can temporarily lower testosterone. Undernutrition, heavy alcohol use, certain medicines, and disease can also contribute. The laboratory value needs clinical context. 1

What are the concerns with ashwagandha?

The BfR describes possible effects on thyroid and sex hormones and reports of liver injury. It advises particular avoidance in people with current or previous liver disease, children, pregnancy, and breastfeeding. A safe intake has not been established. 3

Which herbs have been studied?

A systematic review included 32 trials of 13 herbs. Only six trials were judged at low risk of bias, and many participants were younger men without a clinical diagnosis. 4

AshwagandhaToo little evidence

Three of four studies reported higher testosterone. The variation in extracts, doses, and participants limits conclusions about treating diagnosed deficiency. 4

FenugreekToo little evidence

Four of six studies reported increases. The review identifies a signal worth investigating, but does not establish a reliable treatment for hypogonadism. 4

How does this differ from prescribed testosterone?

The guideline recommends testosterone treatment for appropriately diagnosed men to correct deficiency symptoms. Treatment requires monitoring and is discouraged when fertility is planned in the near term. 2

Which products are outside this review?

This page does not evaluate DHEA, boron, tongkat ali, ZMA, chrysin, or multi-ingredient testosterone blends.

Questions people ask

Does one low result prove a deficiency?

No. Diagnosis combines symptoms with consistently low results, confirmed by repeat testing. 2

Should every man have screening tests?

The Endocrine Society recommends against routine screening of the general male population. 2

Why is the blood sample taken in the morning?

Testosterone changes during the day; a consistent sampling time makes results comparable. 1

Is an herbal extract a proven replacement for treatment?

The reviewed trials are too heterogeneous and often too small to establish that role. 4

Does natural mean safe for the liver?

No. The BfR cites liver-injury reports involving ashwagandha. 3
