Vitamin B12
Vitamin B12, or cobalamin, is an essential water-soluble vitamin made by bacteria and found almost only in animal foods.
Also listed as Cyanocobalamin, cobalamin, methylcobalamin, 5-deoxyadenosylcobalamin
Replacing a deficiency is a different question from boosting energy
B12 deficiency can cause megaloblastic anemia, in which red blood cells are abnormally large, as well as fatigue and neurological problems such as tingling or numbness. Neurological changes can occur without anemia, and early diagnosis helps avoid irreversible damage. Replacing a missing nutrient addresses this established clinical problem. 1
Will it improve memory or mood without a clear deficiency?
A 2021 systematic review included 16 trials with 6,276 participants without overt B12 deficiency or advanced neurological disease. B12 alone or combined with folic acid, sometimes with B6, did not improve the assessed cognitive domains or depressive symptoms overall. The result applies to those populations, not to treatment of neurological symptoms caused by deficiency. 2
The pooled trials did not show an overall improvement in depressive symptoms. 2
What do we know about unexplained tiredness?
That review found only one study of idiopathic fatigue, meaning fatigue without an identified cause, so the authors could not pool results. This is a research gap, not proof that B12 can never help tiredness. It also leaves little basis for predicting benefit before the cause and B12 status are assessed. 2
Evidence for unexplained fatigue without overt deficiency was too limited for a pooled conclusion. 2
Can tablets replace injections?
A Cochrane review found three small trials involving 153 people with deficiency, followed for three to four months. Two trials using 1,000 µg oral B12 daily found no relevant difference in blood B12 levels compared with injections; one using 2,000 µg found higher levels with oral treatment. 3
The evidence was low quality, and the studies did not report clinical symptoms or quality of life. Restoring a blood level is useful, but these trials cannot show that tablets and injections produce identical neurological recovery. The treatment route still needs to fit the cause and severity of deficiency. 1 3
